Hi Ross,

I've merged your updated patch. I noticed your "FIXME" question about whether to choose header name from backend or frontend. You selected backend only, which looked fine to me. But during tests I found a nasty case which was not covered, it was when only the frontend sets the option, then the backend has no header name. So I changed it slightly so that backend setting has precedence over frontend setting.

I also propagated the change to the IPv6 part.
